Shown in partial cutaway, the male foot and ankle are opened to reveal superficial and deep muscle layers spanning the tarsal and metatarsal regions, with the calcaneus and talus forming the hindfoot beneath the distal tibia and fibula. Plantar muscle groups occupy the inferior aspect, their tendons coursing distally along the metatarsals toward the phalanges, while dorsal tendons lie more anteriorly over the midfoot and forefoot. Posteriorly, the calcaneal (Achilles) tendon inserts on the posterior calcaneal tuberosity, continuous proximally with the triceps surae. Ligamentous bands bridge adjacent bones around the ankle and intertarsal joints, framing the course of neurovascular structures that run in parallel with the major tendons. Layered foot muscle anatomy matters because small differences in tendon trajectory and compartment boundaries drive common pain patterns and surgical decisions. The relationship of the Achilles tendon to the calcaneus underpins Achilles tendinopathy and insertional bursitis, and the plantar musculature and aponeurosis help explain plantar heel pain and the mechanics of the windlass effect during toe dorsiflexion.
