Description
Centered in the mediastinum, the heart sits inferior to the trachea, with the tracheal cartilaginous rings leading into the main bronchi as they pass laterally toward the hila. Blue bronchial veins track along the bronchial wall and accompany the bronchial arteries and bronchi, forming small tributaries that run posterior to the pulmonary vessels and then converge toward systemic venous channels. Red arterial pathways from the aorta and its branches course superior and posterior to the cardiac base, while venous return lines the atrial inflow region, setting up the contrast between pulmonary vessels and the bronchial venous drainage. Bronchial veins matter because they represent the systemic venous outflow of the conducting airways, not the gas exchange bed, and their destinations differ by side: the right bronchial veins typically drain to the azygos vein, while the left bronchial veins often empty into the accessory hemiazygos or left superior intercostal vein.
