Description
Centered in the thorax, the heart sits medial to the paired lungs, with the trachea descending superiorly to the carina and dividing into the right and left main bronchi. Blue bronchial veins track along the bronchi and larger bronchioles, running posterior and lateral to the airway walls and accompanying the red bronchial arteries within the peribronchial connective tissue. At the lung hilum, these small-caliber veins converge toward the mediastinum, draining into systemic venous channels that course adjacent to the great vessels and cardiac base. Bronchial venous drainage matters because it represents the systemic outflow of the conducting airways and helps explain why airway and pleural processes can produce venous congestion or prominent collateral veins near the hilum. The pathway is also a teaching anchor for physiologic shunt: a portion of bronchial venous blood can mix with pulmonary venous return, contributing to the normal alveolar-arterial oxygen gradient even in healthy lungs. In the operating room, this anatomy sits in the dissection plane during hilar control for lobectomy and pneumonectomy, where small bronchial veins can bleed persistently if not clipped or sealed near the main bronchus.
