Description
Centered in the thorax, the trachea descends in the midline and bifurcates into the right and left main bronchi, with lobar and segmental bronchi radiating laterally toward each lung. Blue bronchial veins course along the posterior and inferior aspects of the bronchial tree, running in the peribronchial connective tissue alongside the bronchial arteries and then converging toward the mediastinum. Red great vessels frame the scene, with the ascending aorta and pulmonary trunk superior to the cardiac silhouette and paired pulmonary veins returning to the left atrium anterior to the bronchi near the hila. Close relationships dominate. Bronchial venous drainage is a common point of confusion because it does not mirror pulmonary venous return, and only a portion of bronchial venous blood reaches the azygos system, while the remainder drains via bronchopulmonary anastomoses into pulmonary veins, contributing to the normal physiologic right to left shunt. That distinction matters when you teach why arterial oxygen tension can be slightly lower than alveolar oxygen tension, and when you discuss hemoptysis, where systemic bronchial vessels, not the low pressure pulmonary circulation, often supply the bleeding source. Surgeons and bronchoscopists also rely on the peribronchial vascular pattern when thinking about central airway tumors, post-transplant airway ischemia, and hilar dissection planes.
