Description
Beneath the submental skin of an adult male head and upper neck, the geniohyoid appears as a paired, straplike suprahyoid muscle running from the inferior mental spine (genial tubercle) on the lingual surface of the mandible to the anterior body of the hyoid bone. It lies immediately superior to the mylohyoid and inferior to the genioglossus, crossing the midline as two adjacent bellies separated by a thin raphe-like interval. Posterior to its mandibular origin you can orient to the oral cavity and tongue base, while laterally the mandibular ramus and masseter define the masticatory compartment and provide a clear boundary between suprahyoid and jaw musculature. Orientation is clear. Functionally, geniohyoid advances and elevates the hyoid during the oral and early pharyngeal phases of swallowing, and it can help draw the hyoid anteriorly to assist airway patency when the mandible is stabilized. That relationship matters in dysphagia workups and in post-operative neck anatomy, where suprahyoid weakness or scarring (for example after floor-of-mouth surgery or submental approaches) can reduce hyolaryngeal excursion and contribute to residue or aspiration risk. For procedures that traverse the submental region, appreciating the plane between geniohyoid and mylohyoid also helps explain how infection or hematoma can track toward the hyoid and compromise the airway.
