Description
Layered over the lateral calvaria, the occipitofrontalis (epicranius) is represented through its broad tendinous sheet, the galea aponeurotica, draped superficial to the parietal and temporal bones and continuous posteriorly toward the occipital belly near the superior nuchal line. Anteriorly, the aponeurosis blends with the frontal belly over the forehead and brow, while laterally it approaches the temporal fascia at the superior temporal line. Deep to this scalp layer, the temporalis muscle fills the temporal fossa, its fibers converging inferiorly beneath the zygomatic arch to insert on the coronoid process of the mandible. Cervical vertebrae align inferior to the occiput. Clear landmarks include the zygomatic arch and mandibular condyle. For scalp surgery, this anatomy matters because the galea and loose areolar tissue define the plane used for coronal flaps and explain why scalp lacerations that violate the epicranial aponeurosis tend to gape widely from frontalis and occipitalis tone. Hemorrhage can be brisk. The lateral perspective also supports discussions of the temporal branch of the facial nerve as it crosses the zygomatic arch toward the frontalis, a practical correlation when teaching brow ptosis after facelift or temporoparietal approaches. Pairing the occipitofrontalis with the underlying temporalis helps clarify why temporalis hypertrophy, tendinopathy, or tension-type headache pain is felt at the temple yet transmitted across the scalp via the epicranial aponeurosis. Use this plate in head and neck gross anatomy courses, dental and oral and maxillofacial surgery teaching on temporalis insertion and mandibular mechanics, and operative manuals describing coronal incisions, temporalis fascia harvest, or scalp reconstruction planning.
