Description
Prominent on the lateral aspect of the female thigh, the vastus lateralis occupies the anterolateral compartment of the quadriceps femoris, lying lateral to rectus femoris and anterior to the lateral intermuscular septum and femoral shaft. Proximally, its fibers arise along the greater trochanter and lateral lip of the linea aspera, then converge distally into the quadriceps tendon at the superior pole of the patella, continuing through the patellar ligament toward the tibial tuberosity. Medially, the obliquely oriented sartorius and elements of the adductor group help define the anterior triangle of the thigh, while the fascia lata and iliotibial tract form a firm lateral boundary as the musculature approaches the knee joint. Pelvic landmarks and the patellofemoral region provide bony context for these attachments and lines of pull. Lateral quadriceps anatomy matters most at the patella. A dominant vastus lateralis vector, especially when the vastus medialis obliquus is comparatively under-recruited, contributes to lateral patellar tilt and maltracking associated with anterior knee pain and recurrent patellar instability, and this relationship is easiest to teach when the muscle’s fiber direction is shown against the patellar and femoral landmarks. Surgeons and sports clinicians also reference this muscle for the anterolateral thigh (ALT) flap and for safe intramuscular injection placement in the mid-lateral thigh, where the femur provides a deep backstop and major neurovascular structures remain more medial.
