Description
Posterior inspection of the female thigh places the vastus lateralis on the lateral aspect of the quadriceps femoris mass, with its proximal fibers sweeping inferiorly from the lateral femur toward the quadriceps tendon and patella (anterior to the plane of view). Along the superolateral thigh, the muscle’s origin is understood at the greater trochanter, gluteal tuberosity, and lateral lip of the linea aspera, lying deep to the iliotibial tract and adjacent posteriorly to the long head of biceps femoris. Superiorly the gluteus maximus overlies the proximal femur, while the hamstrings occupy the posteromedial compartment and frame the lateral intermuscular septum. Orientation matters. A posterior perspective is useful because the vastus lateralis is often taught as an anterior compartment muscle, yet its proximal attachments sit close to landmarks that clinicians palpate from behind, including the greater trochanter and the posterior border of the iliotibial tract. In lateral approaches to the femoral shaft (for fracture fixation, biopsy, or decompression), surgeons split or elevate vastus lateralis from the linea aspera and intermuscular septum, and understanding its relationship to biceps femoris helps anticipate where the dissection plane tightens near the posterior compartment. The same anatomy underpins the design of a vastus lateralis myocutaneous flap, where proximal pedicle planning depends on consistent muscle geometry around the lateral femur.
