Description
Anteriorly, the left pulmonary artery is traced as it courses laterally from the pulmonary trunk and gives off the anterior segmental artery to the upper lobe (A3). The vessel runs anterolaterally toward the bronchopulmonary segment, maintaining the typical relationship of pulmonary arteries traveling with segmental bronchi, while the accompanying pulmonary veins lie more intersegmentally and drain toward the left atrium. Superior to the cardiac silhouette, the trachea bifurcates into the main bronchi, providing a midline airway landmark above the great vessels. Color convention distinguishes arterial from venous channels. For teaching and procedural planning, isolating the anterior segmental artery matters because segmental anatomy is what surgeons and interventionalists act on, not just lobes. During left upper lobe segmentectomy of S3, A3 must be identified and divided while preserving adjacent segmental branches (A1+2 and lingular arteries), and confusion at the hilum can translate into ischemia of the wrong segment or troublesome bleeding from an unexpected branch.
