Originating from the left pulmonary artery, the anterior basal segmental artery (A8) courses inferolaterally into the lower lobe, where it distributes along the anterior portion of the basal pyramid. Segmental branches track beside the segmental bronchus and its subdivisions, then taper to arterioles toward the peripheral acinus. Blue pulmonary venous tributaries lie in the intersegmental planes, running more anteriorly and inferiorly toward the left inferior pulmonary vein, while the trachea and main bronchi sit superior to the cardiac silhouette. Segmental arterial anatomy matters when you need to think in bronchopulmonary segments rather than lobes, because A8 often defines the vascular pedicle encountered during left lower lobe segmentectomy or sublobar resection for early peripheral carcinoma. Misidentifying A8 versus the lateral or posterior basal arteries can compromise margins or devascularize an adjacent segment, and venous branches that course between segments can be injured if the intersegmental plane is entered too deeply. This is where preoperative CT angiography maps pay off.
