Description
Anteriorly, the left hilum is presented with emphasis on the apicoposterior vein of the left upper lobe (vena segmenti apicoposterioris), typically formed by tributaries draining the combined S1+S2 bronchopulmonary segment. The vessel courses inferiorly and medially from the superior portion of the left lung root toward the superior pulmonary vein, lying anterior to the segmental bronchi and in close relationship to the left pulmonary artery as it arches posterior to the venous plane. Central landmarks include the left atrium receiving the left superior pulmonary vein, the right and left ventricles, and superficial coronary vessels on the anterior cardiac surface. Proximally, a short segment of trachea and the main bronchi appear superior to the pericardial silhouette. Small, but teachable. Venous anatomy of the left upper lobe matters when you are planning a left upper lobectomy or segmentectomy, because pulmonary veins run anterior and inferior at the hilum and are often the first vascular structures encountered in an anterior fissureless approach. Misidentifying the apicoposterior venous drainage can lead to unexpected venous congestion of the lingula or residual segments, and variants are common, including a shared trunk with the anterior segmental vein or separate drainage into the left atrium. This view also supports interpretation of CT angiography and preoperative 3D reconstructions, where distinguishing arterial from venous branches hinges on their relationship to the bronchi. Thoracic anatomy courses, cardiopulmonary blocks, and surgical skills labs use this perspective to teach the left hilar “vein anterior” rule alongside realistic cardiac orientation and proximal airway anatomy. Medical publishers and device teams can also place this artwork in atlases or operative guides addressing VATS and robotic left upper lobe resections, pulmonary venous isolation, or segmental venous variation.
