Anterior Vein of the Right Lung

Centered in the thorax, the heart sits between the right and left lungs with the trachea descending superiorly to the carina and bifurcating into the main bronchi. On the right side, the anterior vein of the right lung courses from the anterior pulmonary parenchyma toward the hilum, where it converges with other segmental and lobar tributaries to form the right superior pulmonary vein. Anterior to the right main bronchus and closely related to the right pulmonary artery, this pulmonary venous drainage returns oxygenated blood to the left atrium while the pulmonary trunk and its branches deliver deoxygenated blood to the lungs. Relationships at the hilum matter. Clinically, the anterior venous channels of the right upper and middle lobes become relevant during right-sided thoracic surgery and hilar dissection, where the pulmonary veins are encountered in the anterior plane and must be controlled before or during lobectomy. This view also supports teaching of pulmonary venous anatomy as it differs from systemic venous return, a common point of confusion when learners see blue-coded veins adjacent to red-coded pulmonary arteries in mixed cardiopulmonary diagrams. Pulmonary venous variants and short segmental tributaries can complicate isolation of the right superior pulmonary vein in video-assisted thoracoscopic surgery and during left atrial ablation procedures where pulmonary vein ostia are targeted.
