Description

Centered on the anterior aspect of the male knee (genu), the arterial tree is traced from the distal femoral segment into the perigenicular network. The popliteal artery is implied deep to the joint line, giving rise to the superior medial and superior lateral genicular arteries that sweep around the femoral condyles, while the inferior medial and inferior lateral genicular arteries course near the tibial condyles and the margins of the joint capsule. Descending anteriorly, the anterior tibial artery emerges from the popliteal system and continues into the leg, with recurrent branches contributing to the periarticular anastomosis and small arterioles feeding the capsule and adjacent periosteum. Collateral channels from the femoral side, including the descending genicular artery and descending branch of the lateral circumflex femoral artery, complete the proximal inflow to this interconnected ring. Anterior mapping of these vessels matters because the knee’s arterial anastomosis maintains perfusion when the popliteal artery is compressed during flexion or compromised by trauma. This is the roadmap for collateral circulation in superficial femoral artery occlusion and a frequent source of unexpected bleeding in approaches to the distal femur and proximal tibia, including total knee arthroplasty, tibial plateau fixation, and high tibial osteotomy. Small genicular branches also sit in the field for arthroscopic portal placement and for genicular artery embolization performed in refractory knee osteoarthritis pain.
