Description
Arising broadly from the distal half of the anterior humeral shaft and adjacent intermuscular septa, the brachialis occupies the deep anterior compartment of the arm, lying inferior to the deltoid insertion and deep to the biceps brachii. Distally its fibers converge into a stout tendon that inserts on the ulnar tuberosity and coronoid process, placing it medial to the radial neck and directly anterior to the elbow joint capsule. In an anterior presentation you expect the biceps brachii and its distal tendon to cross superficial to brachialis toward the radial tuberosity, with the brachioradialis forming a lateral boundary as the forearm begins. The humerus, ulna, and proximal forearm flexor mass provide the bony and muscular context. For teaching elbow flexion, brachialis is the cleanest “pure flexor” to emphasize because it acts on the ulna regardless of forearm pronation or supination, unlike biceps brachii which gains mechanical advantage in supination. A critical landmark. Clinically, this anatomy supports interpretation of bicipital tendon rupture (preserved elbow flexion from an intact brachialis) and guides safe deep dissection planes during an anterior approach to the distal humerus or elbow, where the musculocutaneous nerve typically traverses the brachialis before continuing as the lateral antebrachial cutaneous nerve.
