Description
Running deep to the biceps brachii, the brachialis occupies the anterior compartment of the arm and arises from the distal half of the humeral shaft, wrapping toward the medial side as it descends to the ulnar tuberosity and coronoid process. From a medial perspective, its belly sits anterior to the humerus and brachial artery course, while the medial head of triceps brachii remains posterior across the intermuscular septum. Proximally, the deltoid caps the shoulder laterally, and distally the brachialis narrows into a short tendon crossing the elbow joint to the ulna. A clean relationship. Brachialis is the primary elbow flexor regardless of forearm pronation or supination, so a medial view is the one that clarifies why loss of biceps function does not eliminate flexion strength. Clinically, its deep position explains why the musculocutaneous nerve (C5 to C7) is tested with resisted elbow flexion, and why distal anterior humeral fractures or aggressive retraction in an anterior approach can compromise that nerve. The muscle also provides anterior coverage of the elbow capsule, a point that matters when teaching anterior elbow pain sources and postoperative stiffness after trauma.
