Description
Arising from the aortic arch, the brachiocephalic trunk (arteria brachiocephalica) courses superiorly and to the right before dividing into the right common carotid artery and right subclavian artery. The ascending aorta and aortic arch sit anterior to the trachea and main bronchi, while the superior vena cava lies to the right of the ascending aorta as it descends toward the right atrium; the inferior vena cava approaches the heart inferiorly. Coronary arteries trace the anterior surface of the heart, and the rib cage frames the mediastinal contents laterally. Classic arterial red and venous blue clarify flow pathways at a glance. This proximal segment is a practical landmark in thoracic surgery and interventional planning because the brachiocephalic trunk sits close to the manubrium and trachea, and its origin can be implicated in arch atheroma, aneurysm, or traumatic injury. Catheter-based procedures often reference the aortic arch branch pattern, and anatomic variants (such as a common origin of the brachiocephalic trunk and left common carotid, the so-called bovine arch configuration) change device selection and cannulation strategy. Orientation matters. Even small shifts in the relationship to the superior vena cava and trachea alter the working corridor.
