Description
Anterior presentation centers on the gastric cardia and its cardiac orifice (ostium cardiacum), where the abdominal esophagus transitions into the proximal stomach at the esophagogastric junction. Superior to the orifice, the distal esophagus descends toward the diaphragmatic hiatus, while inferiorly the lumen expands into the gastric fundus on the left and the proximal body more medially. The lesser curvature begins along the right margin of the cardia, and the angle of His is implied between the left aspect of the esophagus and the fundus. Orientation is straightforward. Clinical teaching often reduces this area to a simple opening, but the anterior view is where you can best explain the relationship between the cardia, the diaphragm, and the anti reflux barrier formed by the lower esophageal sphincter and the acute angle of His. This is the region implicated in sliding hiatal hernia and gastroesophageal reflux disease, and it is also the proximal landmark for Barrett esophagus and adenocarcinoma surveillance at the Z line. Surgeons and endoscopists reference these relationships when describing lesions relative to the gastroesophageal junction and when planning antireflux procedures such as Nissen fundoplication.
