Description
Anteriorly, the superficial anterior compartment of the forearm is opened to emphasize the flexor carpi radialis as it arises from the medial epicondyle of the humerus via the common flexor tendon and descends on the radial side of the volar forearm. The muscle belly lies lateral to palmaris longus and medial to brachioradialis, with the tendon becoming prominent in the distal third of the forearm. Distally it courses toward the wrist, passing deep to the flexor retinaculum in its own fibro-osseous tunnel over the trapezium before inserting at the bases of the second and often third metacarpals. That distal trajectory is the point. Unlike the flexor digitorum tendons that traverse the carpal tunnel proper, the flexor carpi radialis tendon has a separate compartment, a distinction that matters in carpal tunnel release and in explaining why flexor carpi radialis tenosynovitis can mimic median nerve symptoms without true median nerve compression. This anterior view also helps clarify its actions, wrist flexion coupled with radial deviation, and its functional opposition to extensor carpi radialis longus and brevis during controlled grip and lifting.
