Description
Centered on the anterior thoracic wall, the sternum is highlighted against the surrounding adult female skeleton, with the manubrium positioned superiorly, the sternal body extending inferiorly in the midline, and the xiphoid process forming the distal tip. Laterally, the costal cartilages of ribs 1 through 7 approach and articulate with the manubrium and body at the costal notches, while the clavicles meet the manubrium at the sternoclavicular joints. Posterior to the sternum, the thoracic vertebral column and rib heads define the depth of the rib cage, framing the breastbone as the central anterior strut. Sternal anatomy is more than a naming exercise because the manubriosternal joint defines the sternal angle (Angle of Louis), a palpable landmark used to identify the second costal cartilage and count ribs during chest exam, ECG lead placement, and thoracostomy planning. The sternal body also overlies the anterior mediastinum, so midline sternotomy and sternal wire fixation depend on an accurate appreciation of midline orientation and the relationship to adjacent costal cartilages. Small xiphoid variants matter too, since a prominent or bifid xiphoid can be mistaken for an epigastric mass or become a tender point after blunt trauma.
