Paired collateral ligaments flank each distal interphalangeal (DIP) joint of digits II to V, spanning from the medial and lateral tubercles of the middle phalanx head to the corresponding margins of the distal phalanx base. From an anterior (palmar) perspective, the ligaments sit immediately lateral to the volar plate and deep to the fibrous digital sheath, while the terminal portion of the flexor digitorum profundus tendon courses centrally toward its insertion on the palmar base of the distal phalanx. Distally, each ligament tightens as the joint approaches extension, limiting varus and valgus tilt at the fingertip. Small structures, big consequences.
