Description
Centered at the orbital apex, the common tendinous ring (anulus of Zinn) encircles the optic canal and the medial portion of the superior orbital fissure, forming the fibrous origin for the four rectus muscles as they course anteriorly to insert onto the sclera. Superior and inferior rectus lie in the sagittal plane above and below the globe, while medial and lateral rectus bracket it on the nasal and temporal sides, respectively, with their proximal muscle cones converging posteriorly toward the ring. The optic nerve passes posteriorly through the ring’s central opening toward the cranial cavity, closely accompanied by the ophthalmic artery at the canal. A tight anatomical corridor. From a superiorly oriented sectional perspective through the skull base and orbits, this arrangement clarifies why the annulus of Zinn is a high-stakes landmark in orbital apex syndrome: lesions in this region can simultaneously affect vision via the optic nerve and ocular motility via the oculomotor, trochlear, and abducens nerves as they traverse the optic canal or superior orbital fissure. It also frames surgical planning for endoscopic optic nerve decompression and medial orbital apex approaches, where maintaining a safe plane relative to the ring and the optic sheath helps reduce iatrogenic diplopia or vascular injury. The relationship to the connective tissue sheath and muscle cone is the teaching point.
