Description
Seen from a lateral perspective within the male orbit, the common tendinous ring (annulus of Zinn) sits at the orbital apex, encircling the optic canal and the medial part of the superior orbital fissure. From this fibrous annulus, the rectus extraocular muscles arise as a muscular cone, with the superior and inferior rectus positioned superior and inferior to the globe and the lateral rectus coursing anteriorly along the lateral wall toward its scleral insertion anterior to the equator. Posterior to the globe, the optic nerve exits the orbit through the ring, while the bony margins of the sphenoid at the apex frame the annular attachment. A tight space. This view matters because the common tendinous ring is the key landmark for orbital apex surgery and for understanding painful ophthalmoplegia syndromes. Lesions at the apex, including inflammation, fracture, or tumor, can compromise structures traversing the optic canal and superior orbital fissure, producing optic neuropathy and multiple cranial nerve palsies, and the relationship of the rectus origins to the annulus explains why restricted gaze often accompanies reduced visual acuity. It also helps clarify why local anesthetic injection for a retrobulbar block carries risk of optic nerve injury or intraconal hemorrhage when needle trajectory strays toward the annular apex.
