Description

Centered in the orbit, the globe sits anterior to the orbital apex, with the cornea and iris facing forward while the optic nerve exits posteriorly toward the optic canal. Surrounding the posterior half of the eyeball, the four rectus muscles converge proximally toward the annulus tendineus communis (common tendinous ring of Zinn) at the apex, then course distally to their scleral insertions anterior to the equator. Medial and lateral rectus hug the corresponding orbital walls, while superior and inferior rectus lie closer to the roof and floor, framing the intraconal space around the optic nerve. Attention to the common tendinous ring matters because it is the surgical and radiologic landmark for the orbital apex, where the optic canal and superior orbital fissure sit in close proximity to the muscle origins, cranial nerves, and ophthalmic vessels. Compression here produces a characteristic pattern: optic neuropathy plus ophthalmoplegia, as in orbital apex syndrome from inflammation, trauma, or neoplasm. A small misstep also has consequences, since strabismus surgery, orbital decompression, and approaches for optic nerve sheath fenestration all depend on respecting the conal anatomy and the tight relationship between the rectus origins and the optic nerve.
