Centered within the orbital apex, the common tendinous ring (annulus tendineus communis of Zinn) forms a fibrous collar encircling the optic canal and the medial portion of the superior orbital fissure. From an anterior orientation into the left orbit, the ring sits posterior to the orbital rim and deep to the extraocular muscle bellies, where it gives origin to the rectus muscles as they radiate anteriorly along the superior, inferior, medial, and lateral walls. Medially, it lies adjacent to the sphenoid body and the posterior ethmoid region, while laterally it approaches the greater wing of the sphenoid at the fissure margin. Tight space. Big consequences. Surgeons and educators return to this view because the annulus of Zinn defines the crowded transition zone where the optic nerve (CN II) and ophthalmic artery enter the orbit through the optic canal, and where cranial nerves III, IV, VI, and the nasociliary branch of V1 relate to the superior orbital fissure compartments. The distinction between structures passing within the ring (classically CN III divisions, CN VI, nasociliary nerve) versus outside it (CN IV, frontal and lacrimal nerves, superior ophthalmic vein) underpins clinical patterns such as orbital apex syndrome and guides safe corridors during endoscopic endonasal decompression or orbital apex approaches. This is where inadvertent traction, cautery, or edema can turn diplopia into permanent visual loss.
