Description
Within a sagittal cross section of the adult male orbit, the common tendinous ring (anulus tendineus communis) is rendered as a dense connective tissue collar at the orbital apex, encircling the optic canal and the medial part of the superior orbital fissure. From its anterior margin arise the four rectus muscles, superior and inferior rectus positioned superiorly and inferiorly, and medial and lateral rectus flanking the globe on the nasal and temporal sides. Posterior to the globe, the optic nerve courses centrally toward the orbital apex, while the bony orbit frames the scene laterally and superiorly and the adjacent nasal cavity lies medial to the orbital walls. Color coding separates ivory bone from red extraocular muscle and light blue connective tissue and nerve. Clear landmarks. This region matters because the annulus defines the crowded gateway where the optic nerve and ophthalmic artery pass and where cranial nerves III, IV, VI, and V1 traverse the superior orbital fissure in predictable relationships to the ring. Surgical decompression for orbital apex syndrome or traumatic optic neuropathy demands respect for these corridors, and misidentifying the tendinous origin can place the optic nerve at risk during endoscopic approaches to the optic canal or when repairing fractures near the orbital apex. Strabismus surgery also starts here conceptually: rectus muscle paths, pulleys, and innervation patterns make more sense once their shared origin at the ring is fixed in space.
