Medial to the deltoid and biceps brachii, the coracobrachialis runs from the tip of the coracoid process of the scapula to the middle third of the medial humeral shaft, forming a slender muscular column along the proximal arm. Proximally, its tendon blends near the conjoined tendon region with the short head of biceps, while the muscle belly lies immediately anterior and medial to the glenohumeral joint capsule. Distally, the insertion sits inferior to the lesser tubercle and medial to the humeral intertubercular sulcus, aligning with the medial neurovascular corridor of the arm. Medial exposure of coracobrachialis matters because it is a reliable landmark for the musculocutaneous nerve, which typically pierces the muscle belly and can be tethered or injured during anterior shoulder approaches. Entrapment at this point contributes to pain in the anterior arm and sensory symptoms along the lateral forearm, and the relationship becomes clinically concrete during coracoid-based procedures such as the Latarjet, where the coracoid and its attached muscles are mobilized near the joint capsule.
