Description
Sweeping across the anterolateral abdominal wall, the external oblique muscle forms a broad superficial sheet whose inferomedial fibers run in the classic “hands-in-pockets” direction toward the linea alba. Superiorly it attaches to the external surfaces of ribs 5 to 12, then descends to the iliac crest, where its lateral muscular belly transitions into a flat aponeurosis that passes anterior to the rectus abdominis. Medially, that aponeurosis contributes to the anterior rectus sheath and meets its counterpart at the linea alba, while inferiorly its thickened lower border rolls into the inguinal ligament spanning from the anterior superior iliac spine to the pubic tubercle. Surface anatomy is clear. Because the external oblique provides the anterior layer of the inguinal canal, this view maps directly onto common sites of abdominal wall weakness, from indirect inguinal hernia emergence at the deep ring to direct herniation through Hesselbach’s triangle just medial to the inferior epigastric vessels. Surgeons reference the same fiber direction when planning oblique or gridiron-style incisions and when identifying the external oblique aponeurosis to enter the inguinal canal during open hernia repair, where the ilioinguinal nerve often lies on or just deep to this layer. For sports medicine, it also helps localize “side strain” injuries at the rib attachments and along the iliac crest. Anatomy faculty will find it suited to teaching anterior abdominal wall layers and rectus sheath formation in gross anatomy, physiotherapy, and core biomechanics modules, and it also reads well in surgical education materials for inguinal anatomy and hernia approaches. Medical publishers can pair it with labels for the linea alba, semilunar line, and inguinal ligament in atlases or patient-facing abdominal wall guides.
