Description
Seen from the medial aspect of the male lower limb, the femoral artery descends from the femoral triangle along the anteromedial thigh, accompanied closely by the femoral vein, and continues distally toward the knee as it approaches the adductor canal. Proximally, the vessel lies anterior to the femur and deep to the sartorius as it courses within the subsartorial (adductor) canal between vastus medialis laterally and adductor longus and adductor magnus posteromedially. Distal progression brings the artery to the adductor hiatus in adductor magnus, where it passes posteriorly to enter the popliteal fossa as the popliteal artery. A key landmark. Medial visualization clarifies a common point of confusion in teaching and practice: the femoral artery does not traverse the canal alone, and its relationship to the femoral vein changes as the pair spiral, with the vein often lying posterior then posterolateral in the distal canal. That spatial shift matters when planning femoral access, interpreting duplex ultrasound, or considering iatrogenic arteriovenous fistula after catheterization. It also provides a practical frame for understanding why superficial femoral artery occlusive disease often presents with exertional calf pain, while distal embolization or popliteal involvement changes the pattern of ischemia around the knee and leg. Use this medial view in gross anatomy labs, vascular surgery teaching files, and radiology correlation for CTA or angiography of the superficial femoral artery through the adductor canal into the popliteal segment, where lesions and bypass graft targets are frequently discussed.
