Description
Arising from the medial malleolus of the tibia and sweeping posteroinferiorly to the calcaneus, the flexor retinaculum (laciniate ligament) forms a stout fibrous roof over the tarsal tunnel. Deep to it, the tendons of tibialis posterior, flexor digitorum longus, and flexor hallucis longus course from the posterior leg into the plantar foot, with the posterior tibial artery and veins and the tibial nerve running in the same confined space. The calcaneal (Achilles) tendon lies posterior to this region, inserting on the calcaneal tuberosity, while the talus and sustentaculum tali provide key bony contours just deep and inferior to the retinaculum. Spatially, the retinaculum sits medial to the calcaneus and posterior to the medial malleolus, anchoring the transition from leg to foot. Medial exposure of the flexor retinaculum matters because it defines the boundaries of the tarsal tunnel and the points where neurovascular structures are most vulnerable to compression. Tarsal tunnel syndrome classically involves entrapment of the tibial nerve or its plantar branches beneath this retinaculum, often in association with tenosynovitis of the flexor tendons, space-occupying lesions, or hindfoot valgus that tightens the tunnel. It is also a surgical landmark: decompression requires releasing the retinaculum while respecting the posterior tibial vessels and tracing the nerve distally toward the medial and lateral plantar nerves.
