Description
Centered within the middle mediastinum, the heart sits slightly left of the midline with its apex directed inferolaterally toward the left fifth intercostal space and its base oriented posterosuperiorly. On the anterior (sternocostal) surface, the right atrium and right ventricle dominate, while the left ventricle forms the left cardiac border; the anterior interventricular sulcus runs toward the apex. Superiorly, the ascending aorta continues into the aortic arch giving rise to the brachiocephalic trunk, left common carotid, and left subclavian arteries, while the pulmonary trunk emerges anteriorly and bifurcates into right and left pulmonary arteries. Systemic venous return is traced via the superior and inferior venae cavae into the right atrium, with major peripheral arteries and veins extending into the neck, upper limbs, trunk, and lower limbs in a front-facing, semi-transparent female body habitus. An anterior gross anatomy view like this earns its keep when you need to teach surface projections and access corridors: median sternotomy exposes the anterior heart, pericardium, and great vessels in almost exactly this orientation. Coronary artery bypass grafting, aortic cannulation for cardiopulmonary bypass, and emergent pericardiocentesis all depend on a clear mental map of what lies immediately posterior to the sternum and costal cartilages. Short and direct. The spatial relationship of the right ventricle to the anterior chest wall also explains why blunt cardiac injury and right ventricular contusion appear in deceleration trauma, and why cardiomegaly alters the left cardiac silhouette on chest radiography. Cardiology and anatomy faculty can drop this into thorax blocks, cardiovascular physiology lectures, or OSCE stations focused on pulse points, central venous access, and the distribution of systemic circulation.
