Description

Arising from the trigeminal nerve at the trigeminal (semilunar) ganglion, the ophthalmic division (V1) courses anterolaterally in the lateral wall of the cavernous sinus, closely apposed to the internal carotid artery and the abducens nerve (CN VI) within the sinus proper. From its proximal segment, V1 gives rise to the lacrimal, frontal, and nasociliary nerves, which track anteriorly toward the superior orbital fissure, positioned superior to the maxillary division (V2) and anterior to the root entry zone of the trigeminal nerve at the pons. Nearby, the oculomotor (CN III) and trochlear (CN IV) nerves run in the same dural plane, creating a compact neurovascular corridor between the middle cranial fossa and the orbit. Spatial relationships are the point. Clinically, this lateral-to-medial stacking in and around the cavernous sinus frames the classic localization of painful ophthalmoplegia. Cavernous sinus thrombosis, internal carotid artery aneurysm, and cavernous sinus meningioma can compromise V1 sensation (cornea, upper eyelid, forehead) while also affecting CN III, IV, and VI, and the illustration helps explain why an abducens palsy often appears early given CN VI’s intimate relationship to the carotid artery. The proximal course of V1 also matters for percutaneous and endoscopic skull base approaches, where the superior orbital fissure and anterior cavernous sinus are frequent working corridors. Neuroanatomy and head and neck anatomy courses can use this rendering to teach trigeminal divisions in context rather than as isolated branches, and neurosurgical or neuroradiology publications will find it suitable for explaining cavernous sinus syndromes and lesion localization.
