Description
Positioned at the thoracolumbar junction, the male T12 vertebra is shown in continuity with adjacent vertebrae, with the vertebral body lying anterior to the vertebral arch and the spinous process projecting posteriorly in the midline. From the pedicles and laminae, the transverse processes extend laterally, while the superior and inferior articular processes form the zygapophysial (facet) joints at the posterolateral corners of the arch. Posterior element morphology at this level can show faint accessory and mammillary processes, transitional features as thoracic anatomy grades into the lumbar pattern. Intervertebral discs separate the vertebral bodies superiorly and inferiorly. T12 matters because it is where rib-bearing thoracic vertebrae give way to lumbar vertebrae, and that transition changes biomechanics, facet orientation, and surgical landmarks. Clinicians often localize thoracolumbar trauma here, including compression and burst fractures that can narrow the vertebral canal and threaten the conus medullaris and cauda equina, so small posterior process variants can complicate level counting and instrumentation. Pedicle screw trajectories also change at this level. Small details can prevent wrong-level surgery. Teaching and publishing use cases include gross anatomy labs, osteology practicals, and spine modules that contrast thoracic versus lumbar vertebrae, as well as surgical anatomy references for posterior approaches and thoracolumbar fixation planning.
