Description
Rising from the superior mediastinum, the common carotid arteries ascend bilaterally within the neck and bifurcate near the upper border of the thyroid cartilage into internal and external carotid arteries, while the internal jugular veins run lateral to the carotid arterial tree in the expected carotid sheath relationship. Superficial to that deep axis, the external carotid gives off facial, lingual, occipital, posterior auricular, maxillary, and superficial temporal branches that fan across the face and scalp, with companion venous channels draining toward the retromandibular vein and back into the internal jugular system. A translucent cranium permits appreciation of intracranial inflow via the internal carotid and vertebral arteries and outflow via the dural venous sinuses into the sigmoid sinus and jugular bulb. Fine arterial and venular branches, down to capillary beds, interlace throughout the cranial and cervical soft tissues. Carotid sheath anatomy matters in the real world because it is the corridor of carotid endarterectomy, central venous access, and deep neck infection spread, where a few millimeters can separate safe dissection from injury to the internal jugular vein or carotid artery.
