Presented in cross section through the orbit, the globe sits within the bony orbital cavity with the sclera shown near its equator, where multiple vorticose veins (vortex veins) converge from the choroid and exit obliquely through scleral emissary canals. Extraocular muscles sleeve the globe anterior to posterior, while the optic nerve leaves the posterior pole and courses toward the optic canal at the orbital apex, accompanied by the central retinal vessels. Surrounding skull structures outline the medial orbital wall and adjacent nasal cavity, with recognizable bony ridges and openings that correspond to the ethmoid region and related paranasal spaces. Vorticose veins form the dominant venous outflow of the choroid, and their clustered, quadrant-based exit points explain why suprachoroidal hemorrhage can track posteriorly and why venous congestion may appear segmental in choroidal imaging. This relationship matters in ocular surgery. During scleral buckle placement or transscleral approaches for uveal melanoma and choroidal drainage, an unrecognized vortex vein at the equator can be avulsed, producing brisk bleeding that is difficult to tamponade in the confined orbital space.
