Description
Running as the most lateral column of the erector spinae, the iliocostalis lumborum is visible deep to the superficial back muscles in a posterior view of an adult male. Its broad, fleshy origin spans the dorsal sacrum and posterior iliac crest, blending with the thoracolumbar fascia, then courses superiorly and slightly laterally toward the lower ribs, lateral to longissimus and separated from it by a fascial plane. Superiorly, trapezius and posterior deltoid contour the shoulder girdle, while latissimus dorsi sweeps inferolaterally toward the humerus, partially overlapping the lumbar region; inferiorly, gluteus maximus covers the posterior pelvis with the hamstrings and gastrocnemius continuing distally in the lower limb. Muscle fiber direction is clear. Because iliocostalis lumborum anchors the lumbar spine to the rib cage, this posterior relationship to the sacrum, iliac crest, and thoracic wall is the teaching point for lateral trunk extension and ipsilateral side-bending. Clinically, the muscle is a common generator of mechanical low back pain and paraspinal trigger points, and it is frequently strained at the thoracolumbar fascia during heavy lifting or repetitive lumbar extension. Its lateral position also matters during posterior approaches for lumbar procedures, where dissection through the erector spinae compartment and retraction planes can contribute to postoperative paraspinal denervation, atrophy, and persistent pain when the segmental dorsal rami are irritated.
