Description
Arising from the anterior part of the orbital floor on the maxilla, the inferior oblique muscle sweeps posterolaterally beneath the globe, then curves superiorly to insert on the posterolateral sclera in the temporal quadrant. From an anterior view through the orbital aperture, its belly is seen coursing lateral to the nasolacrimal canal and inferior to the inferior rectus, with the tendon disappearing toward the lateral wall near the zygomatic bone. Surrounding bony landmarks include the frontal bone forming the superior orbital rim, the maxilla and infraorbital foramen inferiorly, and the nasal bones medially. Short, muscular, and distinctly arced. That curvature is the teaching point. The inferior oblique is the only extraocular muscle that originates anteriorly on the maxilla rather than from the common tendinous ring, which explains its characteristic action pattern, extorsion with elevation when the eye is adducted, and why it is spared in many orbital apex processes that affect the oculomotor-innervated recti. Clinically, this anatomy underpins interpretation of oblique dysfunction in congenital superior oblique palsy with secondary inferior oblique overaction, and it guides strabismus surgery, where inferior oblique recession or myectomy is performed near its insertion while respecting the adjacent inferior rectus and the course of the inferior division of the oculomotor nerve.
