Description
Anteriorly, the vertebral bodies stack in the midline with the intervertebral discs interposed, while the transverse processes project laterally on both sides across the cervical, thoracic, and lumbar levels. Between adjacent transverse processes, the intertransverse ligaments appear as scattered, variably developed fibers spanning the intertransverse spaces, positioned lateral to the vertebral bodies and medial to the most lateral extent of the transverse processes. Their segmental pattern contrasts with the continuous longitudinal ligament bands that run superior to inferior along the column. Small and irregular by design. Clinical teaching often overlooks these ligaments because they are inconsistent, yet they matter when you are localizing the lateral boundary of the intervertebral foramen and the working corridor for posterior and posterolateral approaches to the spine. In the lumbar region, the intertransverse ligament blends with the thoracolumbar fascia and sits near the exiting nerve root, a relationship that becomes relevant in transforaminal epidural steroid injection technique and in far-lateral disc herniation exposure. Expect regional variation: the cervical intertransverse tissue may be minimal or replaced by intertransverse muscles, while lumbar fibers tend to be more discrete.
