Description
Posteriorly, several mid thoracic vertebrae are presented with the spinous processes aligned in the midline and the laminae and transverse processes projecting laterally on either side. Intertransverse ligaments are highlighted as short fibrous bands spanning the gap between adjacent transverse processes, positioned lateral to the vertebral canal and deep to where the intrinsic back muscles would normally overlie them. The costotransverse region is implied by thoracic morphology, with the transverse processes forming the lateral anchor points for these segmental restraints. Small structures. Tight spacing. This posterior perspective matters because the thoracic intertransverse ligaments contribute to segmental control of lateral flexion and help resist shear between vertebrae where rib attachments already limit motion, a useful contrast to the looser, more variable intertransverse tissues in the cervical and lumbar regions. Clinically, these bands sit in the corridor traversed during posterior approaches to the thoracic spine and can be encountered when elevating the erector spinae and transversospinalis muscles off the transverse processes, or when orienting for pedicle screw trajectories where lateral bony landmarks dominate the field. Their relationship to the transverse processes also helps learners avoid confusing them with the deeper ligamenta flava, which bridge adjacent laminae closer to the spinal canal.
