Description
Running between adjacent transverse processes of the lumbar vertebrae, the intertransverse ligaments form paired connective tissue bands positioned lateral to the vertebral bodies and intervertebral discs. Posteriorly, they sit near the zygapophysial (facet) joints and blend with the deep paraspinal fascia; anteriorly, they lie close to the psoas major and quadratus lumborum margins. Their segmental pattern from L1 to L5 is read against the larger lumbar transverse processes and the intervening blue intervertebral discs, with the intervertebral foramina and exiting spinal nerves implied just medial to the ligamentous plane. Texture matters here. Regional variation in intertransverse ligament thickness is not an academic footnote, it explains why “ligament” can behave like a membrane in the lumbar region and more like a distinct cord in parts of the thoracic spine. In the lumbar spine these structures limit contralateral lateral flexion and contribute to segmental stability during rotation, so disruption in trauma or laxity with degenerative change can alter facet joint loading and amplify mechanical low back pain patterns. Their lateral position also makes them relevant landmarks during posterior and posterolateral approaches, where the surgeon navigates between multifidus, longissimus, and quadratus lumborum to reach the transverse processes, pars interarticularis, or lateral recess.
