Arising from the transverse processes of the thoracic vertebrae, the lateral costotransverse ligaments span laterally and slightly inferiorly to the nonarticular part of the rib tubercles, reinforcing the costotransverse joints from a posterior perspective. Adjacent structures expected in this view include the laminae and spinous processes medially, the rib necks and angles laterally, and the posterior intercostal muscles occupying the intercostal spaces between successive ribs. Fibrous bundles sit deep to the superficial back musculature and lie posterior to the costovertebral joint capsules, forming a restrained but distinct band at each thoracic level. Clear landmarks. Posterior exposure of the costotransverse region matters when you need to distinguish pain generated at the costotransverse joint from costovertebral or paraspinal sources, a common problem in thoracic axial pain and post-traumatic rib dysfunction. These ligaments limit excessive motion of the rib relative to the transverse process, so tearing or sprain can follow rotational injury, and hypertrophy or scarring may contribute to localized tenderness that is reproducible with rib springing or deep inspiration.
