Description
Arising from the posterior thoracic wall, the lateral costotransverse ligaments span from the nonarticular part of each rib neck to the tip of the corresponding thoracic transverse process, forming a short, obliquely oriented band just lateral to the costotransverse joint. The thoracic vertebrae are presented in lateral profile, with the rib heads and necks positioned posterolaterally and the rib shafts curving anteriorly to define the thoracic cage. Between adjacent ribs, intercostal musculature occupies the intercostal spaces, while costal cartilages are indicated anteriorly at the rib ends. A compact stabilizing complex. Functionally, these ligaments restrain excessive gliding at the costotransverse articulation and help guide rib motion during respiration, a point that becomes clear when you compare upper ribs, which favor a pump handle pattern, with lower ribs, which contribute more to a bucket handle excursion. Their relationship to the transverse processes and rib necks also matters when teaching the posterior approach to thoracic paravertebral block or costotransverse ligament puncture techniques, where the needle target lies near the transverse process and the costotransverse complex. Degenerative change, ankylosis, or traumatic disruption around the costotransverse joint can limit thoracic cage compliance and contribute to focal posterior chest wall pain.
