Description

Medial exposure of the male lower extremity brings the tibial shaft into sharp relief, with the deep posterior compartment arranged immediately posterior and lateral to its medial border. Tibialis posterior and flexor digitorum longus track along the posterior tibia, while flexor hallucis longus sits deeper and more lateral, approaching the ankle behind the distal tibia. Superficially, the soleus and the medial head of gastrocnemius form the posterior contour of the calf and converge inferiorly toward the calcaneal (Achilles) tendon. Clear layering. Teaching the medial leg through this relationship to the tibia pays off when you need to explain tendon paths and neurovascular corridors at the ankle. The tibial nerve and posterior tibial vessels course deep to the flexor retinaculum in the tarsal tunnel, a common site of entrapment neuropathy that produces plantar paresthesia and intrinsic foot weakness. Posterior tibial tendon dysfunction also makes more sense from this angle, because the tendon’s position posterior to the medial malleolus explains both its mechanical role in supporting the medial longitudinal arch and its vulnerability to tenosynovitis.
