Description
Dominating the right upper quadrant, the pediatric liver fills the subcostal region beneath the right hemidiaphragm and extends medially across the epigastrium toward the left hypochondrium. Its anterior surface sits deep to the lower costal cartilages and abdominal wall, with the inferior margin approaching the right costal margin and the superior border rising toward the dome of the diaphragm. A faint suggestion of the falciform ligament would be expected along the anterior midline, separating the larger right lobe from the smaller left lobe as it courses toward the umbilical region. Pediatric anatomy makes liver topography worth getting right. In children the liver is proportionally larger than in adults, so the inferior edge can be palpable below the costal margin even in health, a point that influences bedside assessment for hepatomegaly in hepatitis, congestive hepatopathy, or storage disorders. Surface localization also guides hepatobiliary imaging and interventions, from ultrasound windows through the intercostal spaces to planning percutaneous biopsy trajectories that avoid the pleura superiorly and the colon and gallbladder inferiorly. Palpation landmarks matter. Use this rendering when you need a clear external-to-internal correlation for pediatric gross anatomy labs, clinical skills teaching on abdominal examination, or textbook figures introducing hepatobiliary anatomy and common pediatric liver enlargement patterns.
