Description
Across the anterior female thorax, paired mammary glands sit superficially over the pectoralis major fascia, extending from the parasternal region medially toward the anterior axillary line laterally, and from roughly the level of the second to sixth ribs superior to inferior. Converging lactiferous ducts course from lobules within the glandular parenchyma toward the nipple, widening into lactiferous sinuses deep to the areola, while a surrounding layer of skin and subcutaneous fat defines the breast contour. Deep landmarks include the clavicles superiorly, the rib cage and costal cartilages beneath the breasts, and portions of the thoracic vertebral column posteriorly, with the upper abdominal viscera (liver on the patient’s right, stomach and intestines inferiorly) providing inferior context. A thyroid gland appears in the lower anterior neck. An anterior breast view with ductal architecture is the workhorse for teaching lymphatic drainage and the routes of tumor spread, because it reinforces how glandular tissue overlies the pectoral fascia yet communicates laterally toward the axilla. Surgeons rely on these relationships during mastectomy and sentinel lymph node biopsy, where the plane between breast tissue and pectoralis major must be respected while preserving or intentionally removing tissue at the axillary tail (of Spence). Duct-centered pathology also reads clearly here: ductal carcinoma in situ follows the lactiferous tree, and subareolar abscesses and periductal mastitis cluster around the nipple-areolar complex. Key surface landmarks.
