Description
Arising from the lateral aspect of the dorsal venous network of the foot, the lateral marginal vein courses posterior to the base of the fifth metatarsal and continues proximally along the lateral border toward the retromalleolar region. Superficial to the extensor tendons and the intrinsic dorsal musculature, its path contrasts with the medial marginal vein, which ascends anterior to the medial malleolus toward the great saphenous vein. Bony landmarks such as the calcaneus, talus, lateral malleolus of the fibula, and metatarsals provide fixed reference points for the vein’s relationship to overlying skin and underlying tendons. Venous anatomy along the lateral foot matters when you are mapping superficial drainage into the small saphenous vein, or when distinguishing normal variants from post-thrombotic collateral channels. Injury risk is real. The lateral marginal vein lies in the field of common lateral hindfoot and ankle incisions, and its proximity to the sural nerve as both pass posterior to the lateral malleolus explains why patients can develop postoperative dysesthesia or painful neuroma if the superficial plane is handled carelessly. The same territory becomes clinically conspicuous in chronic venous insufficiency, where reflux into the small saphenous system can produce varicosities tracking up the posterolateral calf.
