Description
Centered on the scalp and forehead, the occipitofrontalis (epicranius) is highlighted with emphasis on the frontal belly lying superficial to the frontal bone and superior to the supraorbital margins. Posteriorly, the occipital belly occupies the occipital region, and both bellies meet across the vertex through the galea aponeurotica (epicranial aponeurosis), a broad tendinous sheet tethered to the skin and superficial fascia of the scalp. Deep to the muscle-aponeurotic layer, the semi-transparent skull provides bony landmarks, including the orbits, nasal aperture, zygomatic bones, and mandible; inferiorly, the upper thoracic cage is faintly present to anchor overall orientation of the head on the trunk. Spatial relationships are clear. Skin sits immediately superficial. For clinical teaching, this view is about the scalp as a functional unit rather than an isolated facial expression muscle: contraction of the frontal belly elevates the eyebrows and produces horizontal forehead rhytids, while the occipital belly stabilizes the galea so the frontal belly can move the scalp anteriorly. Surgeons and injectors care because the frontalis is the primary elevator of the brow and is commonly targeted with botulinum toxin, where overly inferior or excessive dosing can contribute to brow ptosis or eyelid heaviness through unopposed depressor activity. The layered presentation also reinforces why scalp lacerations that violate the galea can gape widely and bleed briskly, and why the temporal branches of the facial nerve are at risk with incisions or trauma near the lateral forehead.
