Running along the lateral aspect of the anterior forearm, the paired radial veins (venae comitantes) flank the radial artery from the distal arm into the wrist, maintaining close apposition to the vessel within the deep fascial compartment. Proximally, the brachial artery continues from the axillary artery, with its companion deep veins, before dividing in the cubital fossa into the radial and ulnar arteries with their corresponding venae comitantes. Superficial venous pathways are also represented, including the cephalic vein on the lateral (radial) side and the basilic vein on the medial (ulnar) side, set against the osseous framework of scapula, humerus, radius, ulna, and the metacarpals and phalanges. Bones anchor the map. Seeing the radial veins in an anterior orientation matters because their relationship to the radial artery and distal radius is the one you have in mind during transradial arterial access, open reduction and internal fixation of distal radius fractures, and decompression procedures around the volar wrist. Small as they are, these venae comitantes can be a source of troublesome bleeding when the artery is exposed, and their close coupling to the artery explains why edema, scarring, or a tight fascial compartment can impair venous outflow in acute forearm compartment syndrome. The cephalic vein provides a useful contrast, highlighting the superficial system typically chosen for cannulation versus the deep system that follows the arteries. Use this plate in upper limb anatomy teaching to reinforce the distinction between superficial veins and deep venae comitantes, and in surgical or interventional publications discussing radial artery access, wrist approaches, or vascular injury patterns in the forearm.
