Description
Running vertically within the rectus sheath, the superior epigastric vein is traced on the anterior abdominal wall as it descends from the internal thoracic (mammary) venous system and courses inferiorly behind the costal margin. Its tributaries and accompanying venules lie deep to rectus abdominis and near the linea alba, while more lateral thoracoabdominal veins track along the anterolateral trunk toward the axilla. Superiorly, the vessel’s relationships to the diaphragm and the falciform ligament region are emphasized, placing it in the same anatomic neighborhood as venous channels that communicate with hepatic and diaphragmatic veins. Clinically, this is the venous pathway surgeons and interventionalists keep in mind when working near the upper midline abdomen and lower anterior thorax, because it forms a key connection between the internal thoracic system and the epigastric venous network. Portosystemic collateral flow can recruit superficial and deep epigastric channels, and dilated paraumbilical veins may become apparent when portal hypertension redirects blood toward the thoracoepigastric and internal thoracic routes.
