Description
Positioned at the thoracolumbar junction, the T11 vertebra is presented as a single thoracic element with its vertebral body anterior to the vertebral arch, and the spinous process projecting posteriorly from the fused laminae. Short, relatively rudimentary transverse processes extend laterally from the pedicle lamina junction, while the superior and inferior articular processes sit posterolaterally to guide zygapophyseal joint alignment. The vertebral foramen lies centrally, bounded by pedicles laterally and laminae posteriorly, and the costal facets for rib articulation are reduced compared with mid thoracic levels, reflecting the transition toward lumbar morphology. T11 matters because it sits where thoracic kyphosis begins to give way to lumbar lordosis, and that change in facet orientation and process morphology influences motion and load transfer. Clinically, this is a common level for thoracolumbar compression fractures in axial loading and flexion injuries, and it is also a reference point when planning posterior instrumentation that spans the thoracolumbar junction. Small transverse processes can be a practical pitfall when trainees attempt to use them as palpation or fluoroscopic landmarks for level identification. Know your landmarks.
