Description
Beneath the palmar skin of an adult male hand, the four lumbrical muscles arise from the radial (lateral) sides of the flexor digitorum profundus tendons to digits II through V, lying anterior to the metacarpals and deep to the palmar aponeurosis. Each lumbrical courses distally and slightly laterally toward the proximal phalanx, then swings dorsally along the radial aspect of the finger to blend with the extensor expansion (dorsal digital hood). Around them, the phalanges, metacarpals, and carpal bones set the scaffold, with metacarpophalangeal and interphalangeal joints aligned in series. Small muscles. Precise mechanics. Lumbrical anatomy matters because these muscles couple flexion at the metacarpophalangeal joints with extension at the proximal and distal interphalangeal joints via their insertion into the extensor apparatus, a relationship that explains classic postures such as ulnar clawing after low ulnar nerve palsy (digits IV and V) versus a combined median and ulnar palsy. Their close association with the flexor tendons also makes them relevant in trigger finger surgery and in inflammatory tenosynovitis, where tendon sheath pathology can secondarily alter intrinsic balance. In the operating room, understanding their radial-sided course helps when working through palmar approaches near the A1 pulley and when interpreting intrinsic tightness tests at the MCP joint.
